Just fill in your address here over at the Chicago Board of Election and you'll get the ballot for your precinct. Here's an example of a sample ballot to be seen in 6th Ward precincts, 21, 22, & 64. In addition this ballot will also been seen in 7th Ward precincts 36, 37, & 43; 8th Ward precincts 32, 34, 39, 47, & 66; and 10th Ward precincts 02, 03(A), 05, 15, 16(A), 17, 21, & 42(A). Also note the corrected con-con question on this ballot.
